New Patient Registration
We provide services to patients in the NG7, NG8 and parts of NG1 areas.
If you live in the area covered by the practice and are going to be here long term you are welcome to register. Please provide two forms of ID (one photo ID and one proof of address such as a utility bill, bank statement or tenancy agreement).
The reception staff will ask you to complete a small form with your details and then enter your details into the system.
Once registered, you are welcome to book an appointment with the healthcare assistant for a routine new patient health check.
Medical treatment is available from the date of registration. Please contact reception for further information.

Dr Amin is the named accountable GP at St Luke's Surgery, however you are welcome to see any of the GP's within the practice
